WHY WE SHOULD STAY WITH THE MEDICAL SOCIETY


For those of you who do not know me, my name is Maria Basile.  I am a colon and rectal surgeon with a private practice in Port Jefferson, and have been a member of this medical staff since October of 2000.  I am currently serving a year as Secretary of the Suffolk County Medical Society.  I have been tasked by our leadership, Dr. Schrank and Dr. Kvilekval, to outline the benefits of our Medical Staff’s Institutional Membership with the Medical Society, to help you understand how our Institutional Membership has helped us – individually, as a medical staff, and as a profession.


Individually, this membership has saved me, and at least 500 of you, my colleagues, more than $850 in individual dues to SCMS and MSSNY.


Because of our institutional membership, 4 CME courses were brought to us and professionally taught on site on timely topics of our own choosing, such as


1.       Preparing Your Practice for and Audit


2.       Accountable Care Organizations


3.       Electronic Health Records/ Meaningful Use


4.       Clinical Integration


Future offerings could include


·         ICD-10 Implementation


·         Employment Law


·         Captive PC’s and IPA’s





The MSSNY House of Delegates is my favorite opportunity to see firsthand how issues important to individual physicians can shape and even become Medical Society Policy.  At the Womens’ Caucus  this spring I was exposed to the different ways that female physicians from all over New York State and at varying points in their career still navigate cloaked, hurtful, or thoughtless comments about women in our profession – how they balance home, family, and work-life demands, and explore new models of part time practice or manage reentry into full time practice after maternity, family health, or academic leave.


I witnessed how Dr. Schrank followed issues that spoke to his interest in Sports Concussion through the process of creating resolutions that protect and promote the issues important to doctors and watch them be considered, debated, voted upon, and adopted, or not adopted by the House.


I have sought individual legal support from the medical society for answers about ethical and legal matters, employment contracts, and their ombudsman services against managed care companies’ administrative harassments.





This year our Medical Staff, its leadership and committees, has also sought legal advice from MSSNY.  Senior counsel, Donald Moy was able to attend a meeting of our medical leadership and ad-hoc committee on Captive PC’s to give an opinion on changes to our bylaws to accommodate changing practice models and alignment strategies with our hospitals and what these changes mean in terms of Medical Staff privileges.  We should continue to seek MSSNY’s guidance as we revise our Medical Staff Bylaws to comply with MS 01.01.01 Joint Commission standards.


Because of our Medical Staff’s Institutional membership, MSSNY Legislative Day was improved.  Our Medical Staff and St. Charles Hospital was chosen as one of 9 Remote “Satellite” nodes for live internet streaming of the Physician Town Hall Meeting in Albany, an exciting new format designed to bring Physician Lobby Day to the communities where MSSNY members are practicing.


Because of our pioneering efforts as a Joint Medical Staff taking this leap to join MSSNY as an institution OTHER HOSPITAL STAFFS HAVE FOLLOWED suit.  The entire Albany Medical College Faculty Practice, St. James Mercy Medical Center in Steuben County have joined as Institutional Members, and MSSNY is currently negotiating with 7 other institutions using our model for Institutional membership.


We ARE becoming the change we want to see in this world.


This year Medical Liability Reform has become part of budget discussion at the New York State level.  At the national level, our New York delegation was a vocal contingency to the AMA, pushing them to readopt a stance to have Med Mal Reform as part of the National conversation.  “Med Mal Reform  IS Health Care Reform,” was the message from NY to every other state delegation.


At a recent Legislative Breakfast at Suffolk County Medical Society headquarters, I was so proud to watch our own Dr. Paul Lograno, speaking as president on behalf of his local specialty society,strategize and engage a room full of legislators emphasizing how critical Physician Collective Negotiation would be to level the playing field between physicians and managed care.





These past few weeks thousands of people have been involved in the “Occupy Wall Street” protests in New York City and other cities around the world.  Participants are being roundly portrayed and criticized for being fractured, fringe actors, obstructionist, too individualized in motive, and lacking in a unified message.


OUR MEDICAL STAFF IS DIFFERENT in that we have found a way to use our numbers effectively to serve our priorities.  We HAVE brought a unified voice to issues like Med Mal Reform and Physician Collective Negotiation to and through the NYS Legislature, the Governor’s office and into the National Conversation.
We HAVE found a way to harness all the trust, compassion, stability, and hope that we use each day with our patients and each other to OCCUPY MEDICINE even as we continue to practice medicine.  This way has been with our institutional membership with the Suffolk County Medical Society and the Medical Society of the State of New York.

Don't Wait for ACO's: Call Today to Move Physician's Collective Negotiation Through NYS Legislature

Help level the playing field between solo and small private practice physicians and the large insurance companies that control physician reimbursement.


The Physician’s Collective Negotiations Bill is moving through the Senate and Assembly.  All four of the Suffolk Senators are now co-sponsors and several Assemblymembers are co-sponsors and some are still signing on to the bills.  There is a chance for passage before the Legislature goes home for the summer.  However, all physicians must remain active, and continue to call/fax/email their Legislators  so that they will push Senate and Assembly leadership to bring the bills to the floor for a vote this session.  All Suffolk physicans are asked to call the Senate switchboard and Assembly switchboard to give the following brief message:

Mather Highlights EHR and Meaningful Use Education

The heat is on!  Attestation for Meaningful Use of Electronic Health Records began in April, and the first ARRA Medicare incentive payments to providers were disbursed in May.  Opportunities to learn more about electronic health records and the practice of medicine in this community are coming up over the next two weeks.

·         Thursday, June 9th 12 noon at Jefferson’s Ferry – Mather’s Physician Engagement Team will host its second quarterly EHR Summit / Physicians & Office Manager’s Luncheon.  Join Mather CMIO, Dr. Joseph Ng, other physician colleagues from the community, and me for friendly conversation around best practices in EHR selection and implementation.  Learn what you need to know to help your practice succeed in this time of great change and innovation.

·         Next Thursday evening, June 16th, from 6 pm – 8:30 in CR 1 & 2 at Mather, our Medical Society of the State of New York is sponsoring a 2 hour CME program on Health Information Technology Implementation and Meaningful Use.  Salvatore Volpe, M.D., a  primary care physician, whose practice was one of the first practices in New York State to be recognized as Patient-Centered Medical Home will be the guest speaker.  Also invited is Steven Lisker of CMS who will speak to the Meaningful Use Criteria in ARRA.

Sunrise Clinical Manager Update from CMIO, Dr. Joseph Ng

I hope that everyone is enjoying their SPRING! Its been a long time coming and now everyone's office is probably flooded with allergy sufferers that think they're dying.




Sunrise Clinical Manager is still going full steam ahead as planned, what was not planned is further improvements to the existing system that has been made available to us as purchasers of version 5.5. As you know, my goal and the hospital's goal has always been to improve your workflows and save you time while providing patient safety and cutting edge technology and to that end we have fought a very arduous and conflicting battle to obtain and implement the latest and best into this system so that it's as user friendly as possible. First Impressions is what counts right? What this means to our go-live date is still unknown, but to phrase an analogy (and those that know me know I love them...) Should I get a 2011 model BMW when I know that the 2012 model is a completely different and purportedly better model year? It's taking a lot of decision-making of the pros and cons and self-control to hold back the need for immediate gratification but hopefully we will make the right decision.

Order Sets... Dr. Maria Basile and myself have been hard at work trying to churn them out. If you log into Matherdoc.org, you will see the initial VTE order sets for your opinions and approval. When you log in, there is a tab on the top that is CMIO Corner... click on the link for FORUMS HOME and you will reach the links to the VTE Proph order sets. From now on, that's where we will post the order sets and you can all post your opinions as well under POST REPLY.

Mather Meeting with Stony Brook Radiologists -- "All interested members of the medical staff are welcome to attend."

On Tuesday, April 26, 2011, at 7am in conference rooms 1 and 2 at Mather Hospital,  radiologists from Stony Brook University Hospital are scheduled to meet with Medical Staff Leadership, to give a brief presentation and answer any questions that the Medical Staff Leadership may have.  This meeting is following April's Medical Board meeting during which Mather President and CEO, Mr. Kenneth Roberts discussed having Mather radiology services provided by SBUniversity Hospital physicians.  Administration was asked to set up this morning meeting, and Mr. Roberts is extending an invitation to "all interested members of the medical staff."

Medical Liability Reform by this Weekend

The Governor has asked the Legislature to stay in Albany rather than returning to their home district offices this week to resolve the NYS budget.  Please contact your NYS Assembly members and NYS Senator to support the Medicaid Redesign Team's budget recommendations which include comprehensive medical liability reforms by clicking here .  If you've already sent a letter or email, please CALL your legislators Albany offices today to keep this issue top of mind.  Tell your family, friends, neighbors... you do not have to be a physician to use this site.  The message is clear. MRT recommendations are good for New York, for our doctors, our hospitals, and our patients.  It is a package deal and should not be dissected and dismantled.

We could have meaningful Medical Liability Reform in New York State by April 1st, or even by this weekend, but only if we act, in great numbers, to show support for redesigning the medicaid system now.

Penultimate Physician Team Meeting, Thursday, Feb 3.

Second to last meeting eveyone and this one is mighty important because it will walk us all through the discharge process. Please make every effort to be there as we are nearing the end. We will be wrapping up soon and that's the last meeting, set probably for the week after...

 Its been a fun couple of months and we are nearing the end of the design phase of Sunrise Acute Care. As I have said before we only have tomorrow's meeting and one more and we're done. Tomorrow's meeting will be about the discharge procedure which is relevant to everyone. The goal is to show everyone how simple or difficult this is so that we all know what to expect. I'm hoping that the meeting tomorrow will be short but rest assured this is one of those things that is very important because just like admissions, everyone discharges and this is your chance to comment on it so that we can make the necessary changes to the process. Physician Advisory Council Next Meetings

Work for Mather Hospital's Sunrise Clinical Manager Physician Advisory Council continued through January with weekly Future State Design and Physician-oriented meetings.  Recently, CMIO Joseph Ng had this to say about the work this group is doing...

Today’s meeting went very well in my opinion. It gave everyone a chance to discuss their concerns in some part as well as talk about what they would like to see as part of the system. As I have said before, rest assured I AM taking notes.
The Next meeting will be on Tuesday the 25th of January at 8AM-12PM. I don’t expect the meeting to go on for that long, but I wanted to have that time reserved just in case. Once again, the format of this meeting will be to allow questions as well as comments. I will be sending out this weekend, a list order sets that I have been tasked to build; as well list order sets that are currently on paper. My hope is that prior to the meeting can have an idea as to which of these order sets are superfluous, in which of these order sets are must haves.
Once again, I would like to thank you all for your continuing support for this long and complicated endeavor. I hope that for the next couple of meetings (which I think are the last few) all of you will be able to attend.

MSSNY Blasts Off EHR Meaningful Use Education Sessions

"What Doctors and Office Managers Need to Know to Collect Available Money From Incentive Programs In 2011,"  is the title of a 5 hour educational dinner meeting being held at the Huntington Hilton on Tuesday, February 8 from 3-5pm, and repeated on Wednesday, February 9 at the New York Academy of Medicine.  This, from their flyer:

Registering for the ARRA/EHR Incentive Program, or How I Spent My Snow-Day

Note my newest list of links on the sidebar, designed to help you navigate your way to "Meaningful Use" of your office electronic health record, and more importantly to connect you with the $44,000 of "stimulus money" set aside for each Eligible Professional (EP) who demonstrates meaningful use of his or her EHR.

Perhaps you've read here before how to determine if you are an Eligible Provider.  If you're still not sure if you're an EP or whether you qualify for incentives under the Medicare or Medicaid-based program, click here.

For Step 2, "Register for EHR incentive program,"  the Center for medicare and medicaid services has set up a completely online registration and attestation process.  Tricky, though, you need to have the User ID and password that you created when you first got your (scary organ music) NPI number.

Luckily, you've used this User ID and password recently, maybe even changed your password (you can NEVER change your User ID) or modified some of your profile data ...you remember, when you signed up for or made sure you were enrolled in PECOS!

The good news is there is a great guide to go page-by page, even click-by-click through the registration process on the CMSwebsite, which is turning out to be quite a resource to obtain information about the EHR incentive programs.

Registration began on January 3, 2011, and CMS reported 4,000 registrants in the first four days.  You don't need to have your EHR's certification number (although it is available here if your EHR is certified by the Office of the National Coordinator), you don't even have to have an electronic health record yet to register.   It is expected that many of us are in varying stages of adoption, implementation, and meaningful use of electronic health records, but registration as an eligible provider is just one of the steps you can take now to qualify for ARRA payments.  So why not sign up today?  

Physician Advisory Council Next Meeting -- Documentation 1/13/2011

At this point in Mather's Sunrise Clinical Manager build, decisions must be made regarding Physician Documentation.  This Thursday at 7am-9am is the next meeting of the Physician Advisory Council.  Here's a quick note from CMIO Dr. Joe Ng:

Good Morning everyone. Thank You all for attending the last meeting. I’m sorry that I wasn’t able to make the last meeting but from what I gathered on the phone there were a lot of issues that were raised and some that were solved. This week we will continue on with our design and purposing of physician documentation. I am attaching the Thursday meeting’s powerpoint presentation so that you can peruse it at your leisure, but in summary, we will be demonstrating the forms that we have for physician documentation. We will also have an open forum after both meetings (this and next on documentation) as to how we will use these tools during the first phase of this project. I have also spent some time on looking up medicare guidelines for organ system documentations on physical exams and ROS so that when we click NORMAL on any organ system, we will have what we need to have a properly documented note.
I hope to see you all at this Thursday’s meeting at 7am-9AM at the Sunrise Room#3.

Mather Physician Advisors Continue SCM Future State Design

Seven members of Mather's Physician Advisory Council for implementation of Sunrise Clinical Manager met with members of Mather Clinical Leadership and Future State Design Teams as well as Project Manager Mike Gwardyak, of get vITalized, and design specialists from Eclipsys/Allscripts on Thursday morning to discuss the workflow involved in the DISCHARGE PROCESS.  The discussion was very thoughtful and productive.  Issues that were hashed out and analyzed (not necessarily resolved) included:


  • Discharge orders and Discharge order sets
  • Discharge planning
  • Discharge summaries
  • Prescriptions for medicines to be taken at home and Durable Medical Equipment
  • Prescriptions and nursing homes in an electronic world
  • Home care and ongoing care and follow up
  • Patient education
  • Printing up the "chart"
  • Accessing the record post discharge
The entire Physician's Advisory Council and other interested members of the medical staff are strongly encouraged to attend the next two Thursday morning sessions, Jan 13, and Jan 20 from 7-9 am in the Sunrise Room (3rd Floor, opposite Old Pharmacy), which will focus completely on Physician Documentation.  See how our lives can change...could it be...no more dictating Discharge Summaries?  What about Op notes?  Admission notes?  Consults?  Find out first with the Physician Advisory Council!

HITECH Meaningful Use Reporting Begins and $$$ Disbursed in Two States

Wednesday, the first payments under the Medicaid EHR Incentive Program were issued by Oklahoma and Kentucky. Kentucky processed payment to the University of Kentucky‘s teaching hospital, University of Kentucky Healthcare. The first payment, $2.86 million, was one-third of the hospital’s overall expected amount for participating in the program. Oklahoma issued payments to two physicians at the Gastorf Family Clinic of Durant, OK, for $21,250 each, for having adopted certified electronic health records (EHRs).
